AJFC Injury and Incident Forms
The online Injury Report Form is to be used by Team Officials (Coaches and Managers) or AJFC Committee Members to report an injury to an AJFC player/member.
The online Incident Report Form can be used by any AJFC member to report an incident of any kind at a Club-related activity to the AJFC Executive Committee.
AJFC Payment Reimbursement Forms
Please download the relevant form to record any fees paid to match officials (referees or game leaders) to obtain reimbursement by Abbotsford Juniors Football Club. Completed forms should be sent to the Club Treasurer.
Game Leader Payment Record Form
Downloadable form used by team managers to claim reimbursement for cash payments to Game Leaders.
Match Official (Referee) Payment Record
Downloadable form used by team managers to claim cash payments made to Referees for external competition matches or Associate Referees when acting in place of an absent referee.
