Reporting Incidents At AJFC

The procedures outlined below are for Coaches and Team Managers reporting incidents that occur during a game, training session or other AJFC event. This includes an injury, an incident involving violence or abuse or any other breach of the Club’s Codes of Conduct.

Reporting Incidents causing Injury to an AJFC member

1. Wait to be asked onto the field by the referee or referee’s assistants before entering the field of play

2. Identify the extent of the injury (fracture / strain / sprain requiring medical attention) and where required, request assistance of St John’s if at Campbell Park or call an Ambulance (000) where this is necessary. The Team Manager should seek the assistance of a Ground Official where required.

3. Await diagnosis from any medical professionals present before moving the player or for minor injury, remove the player from the field and obtain ice or other first aid assistance from the home Club

4. Request that referee records injury on their report of the match at end of game

5. The Team Manager is to complete an AJFC Injury Report Form providing as much detail as soon as possible after the match. A copy of the completed form will be sent to the Club Secretary who will log and save a copy of the report.

6. Should an injured player wish to pursue an insurance claim for costs incurred, they are required to complete an online insurance claim and submit within 60 days of date of injury at https://football-nsw.gowgatessport.com.au.

As part of the claims process, the player will be required to complete a Club Declaration form which should be sent to the Club Secretary at secretary@abbotsfordjuniorsfc.com.au for review and sign-off. The details submitted will be compared with the original report and if correct, the form will be signed by the Club and sent back to the player for submission.

Reporting Incidents involving violence or a breach of the Club’s Code(s) of Conduct

1. Ensure safety of any children or minors present – remove from field if necessary. Next, ensure your own safety and well-being. On-field incidents are the domain of the referee and other officials. They are to required to deal promptly with these issues on field. In the event of an Off field incident, seek the support of the nearest Ground Official (whether home or away) who should assist in dealing with the incident. If necessary, contact a local Club Official for further assistance, or finally, make contact a Club Committee member.

2. If the incident involves a physical assault causing injury, the Police are to be contacted (000).

3. Ensure the Referee notes the incident, and agrees to include it on their report on the game and in the digital match sheet in Dribl

3. The Team Manager is to complete an AJFC Incident Report Form as soon as possible after the incident with as much detail as possible. Witness statements should also be sourced where appropriate and submitted with the form. Do this regardless of whether the incident occurred at home or at an away field or venue. Include any identification details of players involved such as shirt numbers. A copy of the completed form will be sent to the Club Secretary.

4. The Club Secretary logs the incident detail and saves a copy of the report. They may refer the incident to the Club’s Disciplinary Committee for consideration and action.

5. The Club Secretary and/or President to report the incident to the Association with appropriate follow-up with Judiciary, if required.

6. Club Secretary or President to provide feedback to the reporter and team officials involved regarding any outcomes or sanctions.
